Form5 Prosthetics provides various 3-D printed prosthetics ranging from standard helper hands/arms to a wide-range of Task-Specific Devices enabling those with limb loss to kayak, bike, and even play an instrument!

 

We will be posting the ‘Device Request Form’ for Summer 2019 at the end of March. Please review the following options to determine which type of prosthesis would be best for you or the recipient.

Wrist-Powered Devices

WRIST-POWERED DEVICES OPERATE BY RESIDUAL WRIST MOVEMENT WHICH OPENS/CLOSES THE FINGERS ON THE HAND. RECIPIENTS NEEDING ANY OF THE FOLLOWING DEVICES’ MUST HAVE SOME IF NOT FULL WRIST MOVEMENT IN THEIR RESIDUAL LIMB.

Elbow-Powered Devices
