Form5 empowers people to successfully interact with their world and their future by 3-D printing and providing eco-friendly, custom-application devices.

As part of our vision to equip individuals with limb differences with the confidence, resources, and support to accomplish anything they can imagine, Form5 is focused on improving and accelerating its work in four areas: 3D printing of customized eco-friendly devices, recycling, developing filament, and conducting CO-FAB Workshops.
